The Team
The Hospital Assessment & Discharge Team undertakes initial assessments, reviews and safeguarding enquiries for vulnerable people and their carers in the hospital. We work with adults over the age of 18 with physical and sensory disabilities and mental health needs.
To provide an excellent customer focused service we use a high-quality, strengths-based approach. This minimises the distress experienced by service users and ensures good outcomes are achieved.
The Role
Working in a busy hospital setting you will be responsible for supporting individuals to manage their needs through the assessment process. All staff members will be assessing people who have been admitted to hospital as well as those in A & E who may or may not be known to Social Care. An essential part of the role will be to work on the wards to assess Kirklees patients and people in A&E.
You will be part of a team who attend multi-disciplinary meetings, this will include link working with particular wards. You will facilitate safe discharges from the hospital, with appropriate support from Deputy Team Managers.
As part of a team, you will respond to urgent and critical situations.
In order to be successful in this role you will:

Have experience of working in a social care environment including offering Information, advice and guidance to people.

Have an awareness of relevant legislation pertaining to Adult Social Care.

Have an understanding of the differing requirements of people with a wide range of communication and other complex needs.

Have a standard of literacy and numeracy to be able to complete financial assessments, keep up-to-date records, write assessment, support plans and reports, correspondence and collate information.

Have an awareness of a range of equality issues and their impact on service delivery.

Work as a member of a team, in a multi-disciplinary setting and build strong professional relationships.

Work across both Dewsbury District Hospital & Pinderfields General Hospital.

Work in partnership with Mid Yorkshire Trust and other partner agencies whilst acknowledging the parameters and restraints within the respective organisations.
Working pattern:
You will be on a rolling rota working 5-days over a 7-day period including public holidays. You will have Thursday off prior to working at the weekend, and Tuesday off after working the weekend.
