UHN is a caring, creative place where amazing people are amazing the world.Job DescriptionUnion: Non-UnionSite: Toronto General HospitalDepartment: Blood Disorders ProgramReports to: Nurse ManagerGrade: D0:08Wage Range: $40.98 - 51.22 per hour (to commensurate with experience and consistent with UHN Compensation Policy)Hours: 37.5 Hours per WeekShifts: DaysStatus: Permanent Full TimeClosing Date: July 30, 2024Job SummaryThe Social Worker for UHN's Blood Disorders Program has a focus on supporting adult outpatients in the Red Blood Cell Disorders (RBCD) clinic.DutiesCollaborate with a core interdisciplinary team to support people living with hemoglobinopathies, including Thalassemia and Sickle Cell DiseaseDemonstrate an excellent understanding of oppression from an Intersectional lens, including but not limited to, discrimination, racism, Anti-Black racism, ableism, disablement, Islamophobia, and xenophobia - which are frequently encountered by patients living with hemoglobinopathies, particularly Sickle Cell DiseaseDemonstrate experience working with these patient communities to address inequities and improve patient experience and outcomesRecognizing that patients who access RBCD clinic are from diverse cultural and ethno-racial heritage and socioeconomic circumstances, outstanding communication skills and work experience with social justice and health equity frameworks will be important characteristics for the successful candidateAssess patients for the purpose of evaluating the need for service, the nature of the problem, and formulating recommendations or treatment plans; implementing an intervention plan with patients, family members, significant others, and the inter-professional team for the purpose of facilitating changes in behaviour, attitudes, feelings and/or the environmentRe-Aeassessing and modifying the intervention plan as requiredLiaising with appropriate community agenciesCompleting applications for services as necessary, establishing contact with members of the inter-professional team and community agencies to obtain, provide or exchange information relating to patients' psycho-social functioning and health care in the past, present and futureRendering services for the immediate benefit of the Hospital and the communityProviding clinical teaching to Social Work Students, Medical Students, Professional Colleagues at University Health Network, Faculties of Social Work and in the communityParticipating in relevant research as well as patient engagement and education activities as well as projects, etc.QualificationsCompletion of a Master's Degree in Social Work requiredGraduate from Schools of Social Work with significant Anti-Oppression and Anti-Racism curriculum, preferredCompletion of undergraduate education focusing on Social Work, Sociology, Critical Race Studies, Ethnic Studies, Disability Studies, and Gender Studies preferredRegistration with the Ontario College of Social Workers and Social Service Workers required. Lived experience as a member of an equity-seeking group, including members of Black and Racialized communities preferredSkilled in providing short-term counselling and case management to meet the needs of large patient volume living with chronic illness and/or disability.
