The Older Persons Mental Health Team assists older people with mental illness to remain living and participating in the community or primary health care setting, whilst also co-ordinating and supporting access to a range of appropriate services to assist with recovery and least restrictive practice.The team supports primary care and aged care providers, solo workers and/or small teams to undertake consumer assessments for a wide range of consumers (including those with complex and unclear aetiology), especially where minimal local consultant support is available.About our Hospital and Health Servicehas 6 hospitals, 2 multi-purpose health services and 4 community health centres that employ more than 3,700 staff.
