Job Description

The Learning Disabilities Care Hub has a new exciting opportunity and are looking for people to join our team to support a new initiative which will improve access to primary care services for people with learning disabilities. The post aims to make improvements which address the inequalities experienced by people with a learning disability to access services; improve the quality of GP LD Registers and access to annual health checks while also playing an important role in supporting people to access national patient safety programmes. The overall objective is to reduce health inequalities experienced by people with a learning disability and to improve their quality of life by the provision of a targeted health support programme extending out to reach those defined as hard to reach working in collaboration with Primary Care. The post will be part of a trust wide team working closely with health inequality leads. The holder will joint work with a wide range of a range of professionals, particularly community learning disability teams. The post involves working directly with service users, families and carers with successful candidates having good communication and ICT skills. We are looking for compassionate and caring individuals to come and join our new team. The role of the Primary Care Assistant Practitioner is to work with the multi professional team and the Registered Nurse contributing to the ongoing assessment, providing and monitoring care in line with the service pathways and as agreed with supervising clinician(s). They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi-professional team and partners within Primary Care. The Primary Care Assistant Practitioner has a breadth of knowledge across the lifespan, providing holistic and person-centered care and support for people of all ages and in a variety of settings. This role will bolster the existing offer to primary care to support a number of key national priorities associated with reducing health inequalities to people with a learning disability. These priorities are: 1. To improve the quality of the GP Learning Disability Register. 2. To support those most vulnerable and hard to reach with access or provision of an Annual Health Check. 3. To support Primary Care with providing people with a learning disability with the offer and provision of the national patient safety vaccination programmes (Influenza and Covid-19). 4. Clinical: To support in improvement programme for GP LD Register. To support Practice Staff in accurately coding people with a Learning Disability onto the clinical system. To support in the uptake and awareness of Influenza and Covid-19 Vaccination Programme. To support in improvements to increase accessibility to services for people within the BAME population who require services. Support in collation of data on barriers / blockages to Annual Health Checks and Vaccination Programmes in order to make Greater Manchester systematic improvements. Supporting Primary Care by relieving pressures being experienced in terms of capacity and demand. Improve health of people with a learning disability with the ability to expedite health conditions (known / unknown) and access into services. Support access and awareness to other services and supporting the delivery of the Greater Manchester Health Inequalities Programme. Strengthen collaborative working between Primary Care Practitioners and Community Learning Disability Teams. Ability to offer, in conjunction with the registered nurse, tailored Health Checks for people who require reasonable adjustments and/or have special interests. Under the supervision and working in partnership with a registered nurse provide and deliver a high standard of nursing care contributing to the ongoing assessment, providing and monitoring care based on evidence and working in partnership with the wider multi-disciplinary team. Provide compassionate, safe and effective care and support to patients in a range of care settings. Monitor the condition and health needs of patients within their care on a continual basis in partnership with patients, families and carer’s, ensuring appropriate escalation to a registered nurse (and/or primary care) when required. Support patients to improve and maintain their mental, physical, behavioural health and well-being. Promote comfort and well-being by ensuring that patients’ personal and social needs are met and be able to care for a patient’s nursing needs. Prioritise and manage own workload, recognising where elements of care can safely be delegated to other colleagues, carer’s and family members. Provide and receive sensitive information concerning a patient’s medical condition. Work with the registered nurse, primary care colleagues and engage with other agencies and community teams to ensure a safe, individualised approach and care plan is in place. Maintain accurate, contemporaneous patient records in line with Trust policy and the NMC Code. Provide effective evidence-based care for service users under the supervision of registered nurses in order to perform non-complex activities, in line with clinical governance and professional standards. AFC 4/1/23JME-263-22 Primary Care Assistant Practitioner Band 4 Communication Communicate effectively with dignity and respect to all patients, relatives and others giving consideration to potential barriers to understanding. Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care. Recognise and report any situations, behaviours or errors that could result in poor care outcomes. Report accidents, incidents, near misses or complaints following Trust policies / procedures. Medicines Management Recognise the effects of medicines, allergies, drug sensitivity, side effects, contraindications and adverse reactions. Undertake safe and effective administration and optimisation of medicines in accordance with Trust policy. Administer medicines via the following routes, oral, topical, rectal, subcutaneous and intramuscular in line with NMC Standards and Trust policies. Monitor the condition and health needs of patients within their care following administration of medication. Recognise and report any situations, behaviours or errors that could result in poor care outcomes related to medicines management. Education and Development Comply with mandatory competencies in line with Trust policy. Maintain own continuing professional development in accordance with NMC CPD requirements and contribute to formulation of own objectives and personal development plan. Support, supervise, teach (where appropriate) and act as a role model to student nurses, healthcare support workers and new staff members, promoting reflection and providing constructive feedback. Contribute to the maintenance of the learning environment. Adopt a reflective approach to own practice with a view to continually improve. Undertake further training as required, prior to carrying out any additional duties identified as relevant to role, to comply with Trust policies and procedures. Research and Development Identify relevant sources of evidence to inform practice. Contribute to surveys, audits, research activity as required to own role, supporting recommended changes in practice.
