Job Description

ABOUT THE ROLE

This role will focus on supporting and empowering individuals to maintain their mental health, also in supporting them to be part of the wider community to help them lead more fulfilling lives. The role will provide pathways, support and appropriate interventions to enable successful resettlements. This role will ensure individuals who are receiving support or require this support, have meaningful conversations about their housing and social care needs.

This role will work alongside the NHS through the ‘Long Term Plan and You’ initiative. This focuses on the long term mental health and social care needs of the population alongside partner organisations. This scheme blends the level of care and support we provide to individuals within the community. The scheme focuses on implementing an approach which sees organisations co-located, co-producing, and co-delivering care and support to the most vulnerable individuals within the community. We are currently recruiting for two individuals, one on a FTC until June 2024 and one on a FTC until September 2024.
