A new opportunity has arisen in the community services of West Essex. The successful candidate will have the opportunity to work between the urgent care response team including the new falls response car and the Epping Integrated community care team.
- Totake responsibility for providing, monitoring & developing the Physiotherapy Service within the west Essex Community: Helping patients to engage as independently as possible in activities which enhance their health & wellbeing.
- This role supports a wide range of presentations supporting both rehabilitation and urgent care pathways in the community setting. This includes seeing patients in their own homes, 24 hour care facilities and clinics. To manage a caseload of patients with complex health & social care needs, using evidence based, client-centered principles to assess, plan, implement & evaluate interventions
- The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Primary Care Aligned Team (PACT) supporting West Essex Primary Care Networks with a focus on prevention of admissions, supporting safe discharge as well as provision for rehabilitation for the West Essex population.
- Flexible working is required. At times this will also include work on a 7 day rota covering 8-8 shifts.
1. To deliver high quality, skilled clinical care to patients at home and provide rehabilitation and long term care/conditions management in a variety of community settings.
2. To manage a caseload of patients with complex health & social care needs, using evidence based, client-centred principles to assess, plan, implement & evaluate interventions
3. To utilise specialist skills and knowledge to holistically assess needs and provide programmes of rehabilitation based on evidence based practice.
4. To work, as required, on a flexible basis to cover all roles required within the team
5. To work within and across professional and organisational boundaries, liaising with GP’s other professionals and statutory and voluntary agencies to deliver individualised and expert clinical care to a high standard to clients.
6. To take responsibility for managing the caseload and the team, in the absence of the band 7
7. To support the Locality Manager/Clinical Lead and colleagues in delivering high quality care that delivers best value, and meets the requirements of commissioners.
8. To be personally responsible for all aspects of safety relating to equipment used when treating the patient or issued to the patient for their own use. To be responsible for checking and/or teaching competency of non-qualified staff in using equipment e.g. walking aids, appliances etc

Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

- Using verbal, non-verbal and written communication tools ensure that patients and carers have an understanding of their condition and the role and aims of Physiotherapy in relation to this. The Physiotherapist must effectively amend their communication techniques accordingly to individual needs of patients (for example the patient may have a learning disability, sensory problems, neurological difficulties or may not use English as their first language).
- To be confident in their own diagnosis and be able to communicate that effectively to others (including medical doctors, other Allied Health Professionals etc).
- To share information in a timely manner with more senior staff if there are any concerns about a patients safety, condition or prognosis.
- To urgently report and take, appropriate action if there are any concerns about staff and/or patient safety to the more senior Physiotherapy staff.
- To represent the community service in meetings with other professionals with an aim of accurately contributing and reporting back relevant information to their line manager.
- Maintain confidentiality at all times, consistent with Trust policy.
- Maintain accurate clinical records in accordance with professional and trust standards.
- Use IT skills accurately in patient notes and report writing.
Professional Skills:
- The Physiotherapist must be able to adapt their communication, treatment, information and advice to meet the needs of individual patients. Patients seen will range from the vulnerable to those who are extremely well informed about their condition.
- The Physiotherapist must be sensitive to patients’ own cultures, beliefs and emotions, especially when asking them to undress for treatment n during assessments or treatments, which are likely to include the use of hands-on physiotherapy skills. Consent must always be obtained prior to assessment and treatment.
- The physiotherapist must take a holistic view of the patients and their wellbeing considering all the factors and not purely focus on a specific medical condition, sign or symptom.
- To be professionally and legally accountable for all aspects of own wok including the management of patients in your care.
- To take a lead for specific projects within your area of work to enhance patient care and service delivery.
- To train, supervise and performance manage junior staff, technical instructors, assistants and students (some on a daily basis) with assistance from more senior members of the physiotherapy team when needed. This will include the use of formal appraisal documentation.
- To effectively manage and prioritise your own workload within the time available including the prioritisation of patients to be treated along with other commitments in the day.
- Actively participate in Continuous Professional Development and maintain a portfolio recording learning outcomes.
- Undertake the measurement and evaluation of your work and current practices and participate in research and development through the use of Evidence Based Practice projects, audit and outcome measures, either individually or with more senior physiotherapists. To make recommendations for change as a result of this process.
- Demonstrate a sound understanding of Clinical Governance and Risk Management and apply this understanding to work situations.
Education and Research:
- Maintain appropriate and up to date knowledge and skills by undertaking continuing education in accordance with personal and service needs.
- Undertake appropriate education and training for professional development, to maintain own knowledge and credibility.
- Identify training needs and provide training and mentoring support/preceptorship to designated team members, pre and post registration students health care professionals and others.
- To provide training and support to clinical colleagues.
- Ensuring that learning opportunities are effectively utilised.
- To actively participate in clinical supervision on a regular basis in line with Trust guidelines.
- To participate in annual appraisal and maintain a personal development plan.
- To participate in research and audit to ensure the development of effective and innovative practice and maintenance of standards.
