A fantastic opportunity has arisen for a motivated Specialist Support Frailty Nurse to join our highly skilled & experienced Early Intervention Team at Cambridge University Hospitals.

The Early Intervention Team (EIT) is a very proactive and innovative multidisciplinary team of Frailty Specialist Nurses, Discharge Planning Specialist Nurses, Occupational therapists and Physiotherapists. The Early Intervention Team has been very well acknowledged for providing proactive Admission Avoidance service, including Frailty screening, initiating comprehensive geriatric assessments for patients admitted to the Emergency Department, Same Day Emergency Care and Short stay wards. The service provides an excellent opportunity to develop quality improvement and service development skills as part of the service expansion.

We are looking for an innovative and motivated team player. You will be working collaboratively with other members of the multidisciplinary team, including our community partners, to provide a holistic, patient-centered admission avoidance service.

You will support timely discharges for patients with long term conditions and complex health needs. You will provide a specialized assessment in order to prevent the unnecessary admissions of patients into acute care or to initiate early discharge planning

Main duties of the job

Promoting admission avoidance and early discharges from the Emergency Department, SDEC, Frailty Unit and Short stay wards, for patients who do not require an acute admission but do require community support/services.

Facilitating safe, effective, and timely discharge through early assessment of patient’s care needs, developing a clear plan, liaising with the members of the Multidisciplinary Team and implementing specialist nursing care programmes.

Responsible for providing specialist advice and support to staff, patients and their family Members/carers
