Applicants are encouraged to self-declare in their application.This position assists with rapid re-housing for individuals experiencing homelessness or those at risk of homelessness. Client services include liaising with landlords including social housing, support in acquiring ID, and finding rental housing. The rapid rehousing team provides short-term ongoing support to promote successful tenancies and individual wellbeing.Position SummaryUnder the direction of the Program Manager, the Community Outreach Worker is, in accordance with professional practice standards, responsible for the following:Arrange and support rental unit viewings, landlord interviews, and lease negotiations.Provide tenant support including education on rights and responsibilities, home visits, and connection to local community resources.Identify, build, and maintain ongoing relationships with local landlords.QualificationsEducationValid Drivers' license and access to a reliable vehicleService Delivery Skills, Expertise and ExperienceMinimum of 2 years' experience working in community, preferably with the program's target populationExperience providing outreach with hard to hard-to-reach populationsCritical SkillsProven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skillsFocus on ResultsDemonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomesAlignment with Organizational ValuesCommitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission, and valuesIndigenous UnderstandingKnowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and familiesInterdisciplinary Team, CollaborationProven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity, and cultural awarenessMCC PhilosophiesStrong knowledge, commitment, and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed, and community development principles in all aspects of the workCommunicationExcellent verbal and written communication skills and proficiency in the use of computer systemsAssetsPost-secondary education in a Social Services fieldAll positions are subject to Criminal Record and Child Abuse Registry checks.
