Bringing together innovative patient care, groundbreaking research and diverse education to help people overcome or recover from disabling injury, illness or age-related conditions Toronto Rehab is located at four sites: University Centre (550 University Ave.), Lyndhurst Hospital, Toronto Rehab's Cardiac and Stroke Centre (Rumsey Road) and the Bickle Centre (Dunn Ave). .DutiesThe Nurse Practitioner (NP), having a specialty-based focus, functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The principal responsibilities include: utilizing and demonstrating a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for complex patients involving collecting and interpreting data about the health of the patient to determine a medical diagnosis and treat complex responses of individuals and families to actual or potential problems; playing a leadership role and performing quality improvement and administrative responsibilities; participating in policy and procedure development; performing research activities; delivering, coordinating and participating in education and professional development activities/programs; performing activities to further develop and strengthen the profession; acting as a resource and serving as a consultant to individuals and groups within the professional community and other hospital/agencies; performing cross-functional and/or other duties consistent with the job classification, with potential career growth and mobility across UHN.QualificationsCompletion of Masters Degree in Nursing required (or one semester from completion)Registered Nurse (EC) certification complete or pendingNurse Practitioner (NP) Adult certificate complete or pendingCurrent Certificate of Competence with College of Nurses of OntarioAdditional education and experience in area of specialty requiredA minimum of 5 years relevant experience including 3 years experience working in acute care medicine; rehab experience an assetExpertise and/or certification in diabetes care, wound careKnowledge of care requirements related to dialysis (hemo and peritoneal), palliative careConfidence in the care of medically complex adults with multiple co-morbiditiesExperience in nursing research, program planning, and evaluation methodologiesEligible for cross-appointment to an academic institution requiredMembership in the Registered Nurses Association of Ontario (RNAO): related professional interest group or association preferredExcellent time management skillsExcellent decision-making, problem recognition and problem solving skillsExcellent negotiation and conflict resolution skillsProgram planning and evaluation methodology skillsCommitment to collaborative practiceAbility to work effectively independently and under stressful conditions.Ability to utilize information technologyAbility to interpret and communicate appropriate policies and procedures to staffAdditional InformationWhy join UHN?In addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks.
