In this role, you will focus on delivering integrated care to vulnerable patients, aiming to prevent hospitalisations and emergency department visits. Working closely with patients, families, carers, and a multidisciplinary team, you will collaboratively set and achieve health goals.Key responsibilities include assessing patient healthcare utilisation, coordinating care, guiding patients through health services, and offering health coaching.
