GENERAL PURPOSEThis role focuses on conducting community assessments and serving as a strategist for inclusive relationships within diverse communities. Responsibilities include collaborative planning, customization, and facilitation of ongoing community conversations. Timely execution of culturally proficient projects and effective communication with stakeholders is essential. The position is key in driving positive change, ensuring fair and just health outcomes, overseeing quality improvement initiatives, and managing reaccreditation with the Public Health Accreditation Board.SUPERVISION RECEIVEDWorks under the general supervision of the Population Health Director and Health Officer.SUPERVISION EXERCISEDSupervises the Community Health Workers.MAJOR RESPONSIBILITIESDirects the work of the bureau within the Population Health Division to ensure that data and community input are used in determining both Population Health Division and Health Department priorities and initiatives.Serve as a strategist to develop and nurture inclusive relationships within diverse communities, specifically addressing health disparities.Conduct comprehensive community assessments to gather relevant data and insights for informed decision-making.Collaborates and coordinates with internal and external partners to ensure that the Health Department and services are delivered with a community-centric focus.Oversee quality improvement initiatives, implementing measures to enhance the effectiveness and efficiency of health programs.Coordinate the reaccreditation process with the Public Health Accreditation Board, ensuring compliance with accreditation standards and continuous improvement in organizational performance.Manages, directs, and coordinates activities through supervisors, which includes team performance, new employee orientation, training, assigning and reviewing work performance, annual work performance appraisal, and discipline.MINIMUM QUALIFICATIONSEducation and Experience: * Education: Graduation from a college or university with a bachelor's degree, major in Public Health, Public Health Promotion and Education, Community Health Education, or a closely related field.Experience: Must have at least two years of work experience in Public Health, Education, Health Education, Public or Health administration, or a closely related field.
