As a registered nurse in this role, you will be responsible for providing nursing care to patients referred to our entry-level Aged Care services. Working alongside a dedicated and goal-driven multidisciplinary team, you will conduct thorough assessments, develop discharge plans, and implement therapeutic interventions tailored to each individual's needs.Additionally, you will also be involved in case management for elderly clients in their homes after their hospital stay. This position offers a Monday to Friday schedule, with an on-call requirement on a rotating roster.
