UHN is a caring, creative place where amazing people are amazing the world.Job DescriptionSite: Queen's EstateDepartment: UHN Transitional Care ProgramReports to: Manager - UHN Transitional Care ProgramWork Model: On-SiteSalary: $27.08 - 33.85 per hour (to commensurate with experience and consistent with UHN compensation policy)Hours: 0.45 FTEShifts: Days/Evenings/Nights/Weekends/HolidaysStatus: Permanent Part-TimeClosing Date : August 1, 2024Please note: This position is a part of the SEIU bargaining unit.Position Summary:UHN Transitional Care Program is located at Queens Estate, provides goal oriented, short-term care to clients from the community, or acute care ALC or at risk of ALC who requires support to complete their restorative process and strengthen their functional capacity to return home or another discharge location. TCP has a total of 34 beds with 3 program streams: Convalescent 20 beds; Reintegration 9 beds; Non-OHIP 5 beds.Duties:Provide education to patients and families regarding relevant deficits and related health promotion interventionsWorks effectively independently as well as collaboratively with inter-professional teamContributes to the care planning process and follows a designed plan of careProvides safe, competent care within Scope of PracticeActively participates in education/mentorship of RPN students and new staffActively participates in professional, program and hospital-wide initiativesCompliance with and commitment to patient safety responsibilities and corresponding hospital plans, policies and procedures in order to ensure a safe environment for patientsQualificationsMember in good standing with the College of Nurses of OntarioDiploma preparation as an RPN in a recognized education program for Nursing preferredCurrent medication certificate (if not included in diploma program)Medical asepsis preferred (if not included in diploma program)Experience in Complex Continuing Care preferredSuccessful completion of Basic Cardiac Life Support (C level) Heart Saver Plus (Canada)Member, Registered Practical Nurses Association of Ontario (RPNAO), preferredWell developed conceptual and theoretical understanding of nursing concepts as they relate to patient population and an ability to articulate these conceptsWell developed clinical decision-making skills that reflect patient centeredness, problem solving and critical thinkingExcellent communication, interpersonal, and leadership skillsAdditional InformationWhy join UHN?In addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks.
