Job Description

Position SummaryEnsure every patient receives a thorough, comprehensive, and individualized physical therapy regimen to ensure optimal patient outcomes.Compensation: $70,000 - $80,000Essential Duties and ResponsibilitiesThe essential functions include, but are not limited to the following:· Perform a thorough evaluation of incoming patients to curate and document a recommended treatment plan based on each patient’s individual needs, consulting with program director and/or referring providers as needed.· Provide exceptional, ongoing care to patients, assisting with all phases of physical therapy treatments and techniques.· Discuss treatments with patients, recording their conditions and responses to therapy in their medical record after each visit.· Complete daily patient care notes, progress notes, discharge notes, and other required forms as requested by referring providers, rehabilitation nurses, or insurance companies.· Collaborate with both scheduling and authorizations teams to help ensure fluid treatment progression.· Operate and apply knowledge of standard physical therapy equipment.· Maintain friendly, cordial relations with all patients, vendors, and colleagues.· Comply with state and Company standards of operations and· Adhere to the Company’s core values and H.O.P.E. mission statement.· Regular participation and measurable involvement in marketing and/or business development efforts.· Additional duties as assigned.Required Qualifications· Master and/or Doctorate degree in Physical Therapy from an accredited program.· Appropriate PT licensure and Colorado state registration (in good standing).· Interest in pain management, including chronic, highly complex patients.Minimum Qualifications (Knowledge, Skills, and Abilities)· 2-3 years with high medical complexity patient populations.· Certification in dry needling is preferred, but not required.· Knowledge of interventional pain management procedures is preferred, but not required.· Excellent oral, written, and listening communication skills.· Effective organization and time management skills with the ability to work in high-pressure situations.· Excellent interpersonal skills and demeanor.· Proficient in the use of a PC; in a Windows environment; in the use of the Internet; and in the use of MS Office Applications such as Outlook, Word, PowerPoint, and Excel.· Proficient in the use of EMR systems such as Modernizing Medicine (“EMA”).Ability to adhere to strict attendance and punctually expectations.Ability to attend to details ensuring accuracy in a timely manner.Ability to work both independently, as well as collaboratively.Ability to identify and proactively solve problems.Ability to thrive in a patient-focused environment.Physical Demands and Work EnvironmentThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
