Trillium Health Partners serves the growing and diverse populations of Mississauga, West Toronto and surrounding communities and is a teaching hospital affiliated with the University of Toronto.Our Mission: A New Kind of Health Care for a Healthier CommunityOur Values: Compassion, Excellence, CourageOur Goals: Quality, Access, SustainabilityOur Enablers: People, Education, Innovation, ResearchPosition Summary:Reporting to the Director of the Cardiac Health Program, the Nurse Practitioner is an advanced practice nurse whose role is comprised of five domains of practice including clinical practice, leadership, collaboration, research and consultation.Supports the Cardiac Health program by performing advanced patient care assessments and coordinating care for patients with complex co-morbidities, ordering appropriate diagnostic tests, pain and symptom management, and patient education in an in-patient settingThe Nurse Practitioner engages in clinical mentoring and/or other academic activities through clinical appointment with an academic institution.The Nurse Practitioner works in a collaborative practice model with physicians, nurses and inter-professional team members to provide quality care to patients and their families within the area of Cardiac Health nursing to achieve quality clinical outcomes through evidence based practice.In partnership with Physician and administrative leadership team develops the policy and practice standards to support one patient experience across the organizationProvides a key leadership role in evaluating quality of care and patient outcomes within the Cardiac Health population, in partnership with physicians and other members of the inter-professionalIdentifies quality initiatives that will strengthen and further develop nursing professional practice within Cardiac Health.The Nurse Practitioner provides leadership and assists with professional development of team members through mentoring and participating in formal and informal education initiatives.Key Qualifications:Minimum masters level nurse practitioner preparation requiredCurrent registration in the Extended Class with College of Nurses of Ontario (Successful completion of Ontario NP-Adult Extended Class examination) required3-5 years clinical experience in the direct care of the acute cardiac adult patients required; cardiac surgery experience preferredMinimum of one year clinical experience in adult cardiac surgery patient management along with a strong interest in the management of cardiac surgery patients requiredPrevious Nurse Practitioner experience in Cardiac Health is requiredStrong problem solving and negotiation skillsMust be able to practice independently and interdependentlyDemonstrated leadership to advance the practice of nursing and inter-professional practice to achieve established goalsExcellent communication skills and the ability to develop positive relationships with patients, families, physicians and inter-professional team membersDemonstrated use of theory and research to enable evidence based practiceExperience with research methodology, program planning and evaluationCurrent Basic Cardiac Life Support certificationCurrent Advance Cardiac Life Support certificationSatisfactory performance and attendance requiredTo pursue this career opportunity, please visit our website:Candidates are selected on the basis of their skill, ability, experience and qualifications.Where these factors are relatively equal seniority shall govern providing the successful applicant.Trillium Health Partners’ (THP) is an equal opportunity employer who values the importance of antiracism work and is committed to integrating antiracism, diversity, equity and inclusion best practices throughout THP operations, policies and culture.
