We have bases at Mona Vale and Royal North Shore and HornsbyWhat you'll be doingProviding acute-level care to patients in the community to prevent hospital admissions, reduce ED presentations and facilitate early discharges from hospitals. As a HITH RN you will be required to assess patients referred to HITH and facilitate transfer to our service., you will also be required to provide clinical care to patient within both our clinic setting and our patient’s homes. Working within a multi-disciplinary team to provide the highest standard of care, we are truly person-centred.People of Aboriginal and/or Torres Strait Islander background encouraged to apply.Candidates will need to meet the following criteria:Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment.
