The Clinical Case Manager will carry a caseload of up to eight clients. This individual's clients will be primarily dual-diagnosed or individuals who have been identified as high-risk clients.Schedule: Monday - Friday 11:00 AM - 7:00 PM (40 Hours)Essential Duties & Responsibilities: * Complete Clinical Intake within the first week of admissionComplete Treatment Plans within first 2 weeks of admission and update as neededBe proficient in the use of the electronic Medical Record for each caseMeet weekly with assigned individualsParticipate in Clinical SupervisionInput entries into the Medical Record must be in line with the service line procedureParticipate in the on-call RotationExpand knowledge and participate in trainingUnderstand DPH regulations and CARF standards as applicableProvide transportation as neededEstablish contacts with DCF, Guardian, doctors and therapists etc. to involve all parties in the treatment processMake outside referrals as neededAttend weekly staff meetingProvide weekend holiday and emergency coverageOther duties as assignedRequired Education/Experience: * A minimum of a High School Diploma or GED is requiredBachelor's degree Preferred or clinical licensure preferredA minimum of five years experience in the fieldIndividual and group counseling experienceMust have an updated MA driver's licenseRequired Skills/Knowledge: * Strong written and verbal skillsAbility to maintain records accurately and efficientlyCapable of responding to crisis situationIf recovering, two years of continuous sobriety requiredAbility to work independently as well as part of a teamPhysical Demands:The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job successfully.
