This position is funded by a grant to enhance the management of patients with cancer in primary care in the Central and Eastern Sydney Primary Health Network (CESPHN). This is a 12-month project based aimed at strengthening integration between general practitioners and cancer specialists and to support GP’s in effectively managing care for their patients with cancer and cancer related needs.Diversity and Inclusion at St Vincent's:St Vincent's is committed to employing a workforce that reflects the community we care for. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia2. Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency and at least 5 years full time post registration experience including in an oncology setting and in addition approved post registration nursing/midwifery qualifications relevant to the field in which he/she is appointed or such other qualifications or experience deemed appropriate by St Vincents Hospital, Sydney3. Demonstrated ability to work autonomously and within the interdisciplinary team and demonstrated commitment to ongoing professional development.4.Demonstrated high level computer literacy with email and electronic patient record systems such as MOSAIQ and HealtheNet5.Demonstrated high level effective communication and interpersonal skills6.Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in their daily practice as a registered nurse7.Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment8.Demonstrated experience in service review and quality improvementShortlisting for this position will commence immediately.
