This involves assessment, treatment and the delivery of individualised interventions utilising an AIMS model approach within the consumer recovery framework.Facilitate evidence-based practice in the area of interprofessional team members of the East Community Care Team to ensure efficiency and effectiveness of service provision and discharge planning, through implementation and facilitation of quality initiatives, literature reviews, attendance at professional development activities and involvement in research and education.Lead and support the delivery of clinical services in accordance with accepted professional practices, standards and principles including the legal and ethical requirements with a high degree of independence.Apply advanced level independent professional judgment regarding the co-ordination and treatment planning of complex patients, as it relates to case management, discharge planning and service transition.Provide operational advice and direction to a team of the East Community Care Team to ensure effective service delivery and quality outcomes applying change management, conflict resolution and negotiation skills where necessary.Find out about the role in more detail and how to apply in the attached Role Description.Our values of Integrity, Compassion, Accountability, Respect, Engagement and Excellence, shape our culture within Metro South Health.
