Remuneration: $54.77 - $56.54 per hour + 11.5% Super + Salary PackagingEmployment Type: Temporary Part Time until June 2025Position Classification: Social Worker Lvl 3Location: Negotiable - Manning Hospital Taree or Forster Community HealthHours Per Week: 24Requisition ID: REQ500487Applications Close: Thursday, 8th August 2024Where you'll be working:The Aged Services LMNCS Transitional Aged Care Program (TACP) is looking for a Social Worker to provide high quality clinical service, as part of the multidisciplinary team, for patients and their families/carers at Lower Mid North Coast Sector (LMNCS).LMNC Transitional Aged Care Program (TACP) is a team of healthcare professionals who help elderly patients improve their health and well-being after leaving the hospital.You will join a supportive team who work together to provide direct support services to patients, families and carers.You will be supported and encouraged to develop and apply creative strategies and solutions to complex patient presentations. A comprehensive orientation, training, clinical supervision and peer support is available for this role.This position can be based at either Manning Hospital in Taree or Forster Community Health Service.About the role:The position is responsible for the overall Social Work service provision to the Aged Care Services LMNCS Transitional Aged Care Program.The team includes Allied Health and Nursing professionals who work closely with patients and their families to enable slow-stream therapy, which improves function, independence, and safety at home.The service is available for home visits, between 8:00 am and 4:30 pm every day, all year round.You will be provided with onsite training to fulfil the role demands including CHIME training.About you:We are looking for someone who is committed to improving outcomes for patients, their family/carers, and assist patients to maintain quality of life.The position will require the successful candidate to show skills such as self-motivation, autonomy, compassion, enthusiasm, and good time management skills in order to complete tasks required each day. The services provided will be dependent on clients’ needs and will vary accordingly.Attributes such as flexibility, the desire to work together as a team and great communication skills, both verbal and written, are required.
