You will be the point of contact ensuring the patient’s journey will be as seamless as possible. You will have a proven track record of written and verbal communication with a wide range of people from different cultural and organisational backgrounds. You will ensure patients receive and can access appropriate care and support based on their individual needs. You will have the right skills in handling data and information between organisations, sectors, patients and carers as this will play a key part in the effective care coordinator roles.
Due to the complex nature of the role, the successful candidate will be required to have good attention to detail, remain calm under pressure and have the ability to show consideration and compassions to others.

- In-depth understanding of national NHS policy and targets including thorough knowledge of the 18 week and cancer pathway targets.
