Job Description

East Lancashire Teaching Hospitals NHS Trust are pleased to offer an 15-month fixed term contract for a Community engagement worker within The Alcohol Care Team. As part of the NHS long term plan Alcohol Care Teams have been funded through NHSE initiatives to provide services to those who use Alcohol and have presented at acute settings. Alcohol Care Teams work in collaboration with partners to: -

The Alcohol Care Team works across Blackburn and Burnley hospital sites. This post is funded by Blackburn with Darwen Council and as such, the post holder will work with people from this locality but will also be required to travel to Burnley to visit Blackburn residents.
1. To build professional and therapeutic relationships with people who use Alcohol and other substances who have attended acute inpatient settings.
2. Work in a flexible and proactive way to engage people with physical and mental health support.
3. Act as an advocate for service users and work collaboratively with partner agencies/stakeholders to identify the barriers which may prevent people from all communities accessing a full range of services, including substance misuse, mental health, primary care and other relevant services.
4. Manage a caseload of people with multiple needs and additional vulnerabilities post discharge including those who frequently use acute settings or emergency services.
5. Facilitate continuity of care for people, liaising with The Alcohol Care team and the wider virtual team into identified local treatment pathways.
6. Work collaboratively, attending multi-disciplinary meetings with partnership agencies, for example: Safeguarding, local treatment service providers, Integrated Neighbourhood Teams and other relevant services.

Communication

7. Provide advice and information through education around the risks associated with Alcohol use, Tobacco and other substances.
8. Use harm minimisation techniques to ensure the safety and well-being of those who use Alcohol and other substances on your caseload.
9. Work collaboratively within a virtual team to address physical healthcare needs such as wound care, the up take of relevant vaccination programmes and testing and treatment for blood borne viruses and sexually transmitted infections.

11. Work with housing providers and support to address housing needs, facilitating appointments.
12. Facilitate access to mental health assessments and treatment as needed.
13. Contribute to the reduction of Drug related deaths through initiatives around harm reduction and overdose prevention.
14. Develop a good understanding of local service provision acting as a resource for those who use Alcohol.
15. Develop and maintain good working relationships with local treatment providers acting as an ambassador for The Alcohol Care team.
16. Conduct home visits and travel to various locations consistent with the needs of those who use Alcohol.
17. Be responsible for the planning and delivery of local events around Alcohol substances and engage with associated initiatives.
18. Take a lead alongside the lived experience worker on and service user involvement.
19. To undertake reasonable requests for additional tasks in response to service requirements to support the wider team as directed by senior colleagues.
20. Provide day to day supervision for the lived experience workers within the team.
