Job Description

We are currently looking for nurses to join our Clapham District Nursing team primarily and other teams around Lambeth and Southwark

We are one of only two community teams in the UK to receive an outstanding status by CQC. Come and join us, and become a part of the amazing work we do!

Guy’s and St Thomas’ NHS Foundation Trust is the first Trust in the UK to trial a new model of District Nursing known as the Buurtzorg Model, an entirely Nurse-led approach to patient care in the community with self managed nursing teams. The post-holder will be a member of the District Nursing Team and will participate in the clinical case management of patients who have a wide variety of complex, high-risk and supported/self-care conditions. Working directly with the Team Leader, the post-holder will help ensure the provision of a high standard of nursing care and health promotion. The care will be provided in a range of settings including patients’ homes, residential homes, nurse led clinics and GP practices.

PROFESSIONAL
1. To adhere to the Nursing and Midwifery Council [NMC] Code of Professional Conduct and be conversant with other relevant advisory papers.
2. To exercise awareness of Professional responsibilities by reading current, relevant literature and by taking up opportunities for personal and professional development in accordance with continuing Professional Development guidelines.
3. To use research based practice to develop and maintain a high quality district nursing service.

MANAGERIAL
1. In the absence of the District Nurse, to take responsibility for delegating the workload appropriately to other team members
2. To assist in the planning of the teams workload. Responsible for providing day-to-day co-ordination of their workload.
3. To be aware of and work with GSTTs’ Policies and Procedures.
4. To assist in monitoring the quality of the district nursing service provided by the team and contribute to the development of quality measures by working closely with the team leader.
5. To maintain statistical and written records as required by the GSTT
6. To undertake performance appraisal of team members, e.g. Senior Nursing Assistants.
7. Attend and participate in meetings across Health and Social Care.
8. Report incidents and accidents in line with GSTT policies.

CLINICAL
1. a] To undertake the first contact assessment, including planning, implementation, evaluation and reassessments of care as part of a case management model of nursing practice.
2. To actively manage patients with long term conditions and work towards preventing complications to reduce the need for hospital admissions.
3. To participate in multidisciplinary working involving other agencies as appropriate.
4. To maintain good working relationships with other health care professionals, statutory and voluntary organisations.
5. To undertake planned and opportunistic health promotion and education activities with patients and carers.
6. Maintains close communication with GP’s and other members of the Primary health care team to promote effective coordination of care.
7. To take responsibility for evaluating and reflecting on his/her own practice.
8. To be actively involved in health screening.
9. Work in partnership with General Practitioners and Practice Nurses to facilitate proactive screening programmes [in line with National Service Framework for Older People] and annual review of the housebound.
10. To perform and maintain competence in a wide variety of clinical skills (which includes but is not limited to) administering injections, catheter management, venepuncture, etc.
11. To actively participate in Nurse-Led clinics within locality e.g. Continence Clinic, Leg Ulcer Clinic, etc.
12. To be aware of the Child Protection Act and Safeguarding adults and to take appropriate action as deemed necessary.
13. To work collaboratively with allied agencies in meeting the needs of clients with long term illnesses.

EDUCATIONAL
1. To participate in the learning programmes for students on community placements.
2. To facilitate the learning of other members of the team.
3. To champion and actively participate in organisational learning and organisational change.
4. To take responsibility for his/her own professional development.
5. To participate in clinical supervision in accordance with community services within GSTT.
6. Act as mentor and/or co-mentor to students and junior staff.
