An exciting opportunity to join our newly formed Home First Rehab & Reablement team. Our service covers the whole of Salford facilitating clients discharge back into their own home. Our vision for Home First is ‘People are in the right place, at the right time - meaning nobody spends more nights away from home than is absolutely necessary’
We are looking to expand our current team and are seeking for caring and compassionate people who enjoys interacting and assisting people in their own homes and assisting them with their daily living activities. You will be an excellent team player and also possess excellent communication skills and must be able to work on your own initiative. Candidates must be warm and friendly, with a strong work ethic, a positive attitude and a genuine passion for providing high quality care and support for the people of Salford.
Due to the role working in the community a Full Driving Licence and the use of own vehicle is essential.
The Home First Reablement Service operates 7 days a week from 8am to 10pm and various shift working is available. The successful applicants will work on a rota that is provided in advance to cover shifts that includes weekends and Bank Holidays.
To provide support and person focused re-ablement as required to help ensure the efficient and effective running of the service.
To provide excellent customer service and establish and maintain communication between individuals, relatives, carers and the multidisciplinary team. To provide holistic patient care, correctly undertake tasks that have been delegated and to record information accurately and pass it on to the relevant people in the team in a timely manner. Duties may include:
- Assist the individual with personal care tasks, such as washing, dressing, eating and toileting
- Assist in the safe lifting, transferring, repositioning and movement of the individual
- To support individuals to live independently in their own homes and help achieve their own outcomes
- Observe, monitor and record clients physical and emotional well-being, and promptly report any changes to senior staff
- Encourage clients to participate in Social and recreational activities
