This is an exciting opportunity for Support Workers to join a one of our semi-independent or supported living homes, for young people aged 16-25 with Mild Leaning Disabilities, supporting emotional, behavioural and learning difficulties .

The successful candidates will have a genuine passion to promote independence for Service Users by working with them to develop their life skills.

The ideal candidate will:

Ø To ensure the well-being of our service users by contributing to the provision of a support service tailored to their individual needs including the promotion of life skills, fostering independence, choice and inclusion in the community.

Ø To work to individual support plans to promote individual life skills including travel training, money management and shopping.

Ø To ensure that accurate records are maintained.

Ø To understand, promote and participate in a multi-disciplinary team approach to provide support to residents

RESPONSIBILITIES

1. To support service users in all aspects of their independent living skills, whilst respecting their dignity and independence.

2. To ensure that service users are provided with the necessary resources to provide a pathway for them to achieve their desired outcome to independence

3. To respond to and meet the identified needs of service users, as outlined in the individual care/support plan.

4. To carry out risk assessments and completion of care/support plans.

5. To contribute to the provision of a safe, secure and positive environment.

6. To monitor progress and ensure that relevant documentation is completed in a timely manner and that accurate records are always maintained.

7. To provide one-to-one mentoring and/or group sessions and organise activities that create interest and stimulate and engage service users, including leisure and other creative activities.

8. To support daily living skills such as budgeting, shopping, claiming benefits, problem solving and engaging the service users in the day to day running of the home, including carrying out domestic tasks associated with normal daily living.

9. To liaise with families, social workers, therapists and other multi-agency organisations, as required to meet service users’ needs.

10. To respect the right of service users to make informed choices, including the right to refuse assistance that does not impact safeguarding.

11. To report to the Team Manager any changes in any of the service users that is not within their support.

12. To report accidents or injuries to the Team Manager immediately and to ensure that there is a written record of support every 30 minutes during the course of each shifts.

13. To work as part of a team and communicate effectively with all staff.

14. To undertake household tasks, administration and data collection as and when required.

15. To promote the strengths of VCS and the support services provided during regulatory visits.

16. To attend and participate in induction, supervision, performance appraisal, staff meetings and training and development programmes, as required.

17. To ensure confidentiality is maintained at all times, in keeping with the Data

Protection Act (DPA) 2018 – ensuring that service users’ personal details are not disclosed to any unauthorised persons and that all records are maintained within the DPA 2018 requirements.

18. To understand and be familiar with all relevant legislation guidance policies and procedures, including those that relate to equal opportunities, safeguarding and health and safety and be able to maintain a best practice approach to ensure requirements are met.

19. To travel between the homes in the West Midlands area, if required.

VARIATION CLAUSE

This is a description of the job as it is constituted at the date shown. It is the practice of this company periodically to examine job descriptions, update them and ensure that they relate to the job performed, or to incorporate any proposed changes. This procedure will be conducted by the appropriate Manager in consultation with the post holder. In these circumstances, it will be the aim to reach agreement on reasonable changes, but if agreement is not possible management reserves the right to make changes to your job description following consultation.
