NHS Continuing Healthcare (CHC) Nurse Coordinators – Registered General Nurse

Permanent

There are full time and part time posts available, we welcome all applications.

Based in Acton House, Worcester or St Owen's, Hereford.

We are expanding our team and are looking for enthusiastic, self-motivated registered nurses to join us.

This is a rewarding and interesting role which will enable you to work in partnership with multidisciplinary teams across Health and Social Care establishing healthcare needs and supporting high quality care for clients and their families in the community.

We are looking for highly motivated practitioners with excellent assessment and communication skills.

Key aspects of the role include:

We are a friendly and welcoming team. You will be allocated a mentor and will be fully supported through your induction using a novice to expert programme. By the end of your first year you will achieve the following key skills;
Working in a CHC service equips professionals with knowledge and experience that will be invaluable to them in their future careers.

Successful applicants will need to be able to work independently to deadlines, have excellent communication skills to engage patients and/or their representatives and be able to produce large, detailed written reports. You will therefore, need to be proficient in the use of Microsoft Word. There is also a continued need to complete remote assessments via Microsoft Teams, confidence with IT systems and leading meetings both face to face and remote is essential.

This a Monday – Friday service, across Worcestershire and Herefordshire, based in one of our offices in either Worcester or Hereford.

Job Summary

NHS Herefordshire and Worcestershire Clinical Commissioning Group was established on 1 April 2020 following a merger of NHS Herefordshire HWICB, NHS Redditch and Bromsgrove HWICB, NHS South
Worcestershire HWICB and NHS Wyre Forest HWICB. We are formed of 83 member GP practices and we are responsible for buying health services for 800,888 people across Herefordshire and Worcestershire.

Specifically, this means we are responsible for:

Internal: HWICB Quality and Patient Safety Teams and Safeguarding Vulnerable Adults Lead.

External: Service Users and Carers, Multi-Disciplinary Team, General Practitioners, Universities, Social Care Services, Domiciliary and Residential Home providers, Voluntary Sector and Acute Hospital staff.

You will need to be a car driver as assessments can take place in client’s homes, community hospitals and care homes across Herefordshire or Worcestershire and out of area where required. There is an opportunity to work flexibly to meet the needs of the service and this includes some working from home.
