Main duties of the job

As a professional in the Health Community team, you will be responsible for coordinating the care the person receives from the Learning Disability service, in line with agreed Care Processes and Pathways. You will work actively with the individual and their families and/or carers in assessing the person’s health needs, developing a Care Plan with them, implementing, and evaluating interventions, and reviewing the expected health outcomes.

We’re an NHS community and mental health provider Trust based in Cornwall and the Isles of Scilly. We deliver community and hospital-based care to improve people’s physical and mental health. We also provide specialist support to people with dementia or a learning disability.

Approximately 568,000 people live here. A third of people who live in Cornwall are supported by acute hospital services in Devon. As a result, we also work closely with our partners in Devon. In the summer, and during other holidays lots of people choose to visit the area. This increases the numbers of people who use our services.

Knowledge and Experience

Essential criteria
  • Relevant clinical experience in a community or health care setting
  • Understand the impact of health inequalities
Desirable criteria
  • Involvement in clinical audit or research projects

Training and Education

Essential criteria
  • Professional Registration with Nursing and Midwifery Council – RNLD
  • Completion of, or willingness to complete, relevant preceptee programme if appropriate
Desirable criteria
  • To act as a mentor for learners. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
