As a professional in the Health Community team, you will be responsible for coordinating the care the person receives from the Learning Disability service, in line with agreed Care Processes and Pathways. You will work actively with the individual and their families and/or carers in assessing the person’s health needs, developing a Care Plan with them, implementing, and evaluating interventions, and reviewing the expected health outcomes.
