Job Description

How you'll make a difference

South Gloucestershire Council are pleased to be partners in the development of the North Bristol Trust (NBT) Transfer of Care Hub (ToCH) and to assist in the co-ordination of safe discharges through a trusted integrated whole system approach. The ToCH will bring together community partners (Sirona), Local Authorities and Voluntary Sector Colleagues across BNSSG to create a new team with multidisciplinary backgrounds. The Hub team will include Acute Trust Case Managers, and Patient Flow Coordinators, Local Authority Discharge Coordinators, Sirona Case Managers, Community OT's, Commissioners, Social Care Practioners, Voluntary Services and Flow and Discharge Coordinators.

The ToCH Team will play a key role in aiding discharge and admission avoidance. The aim is to work collaboratively and holistically, striving to create a person centred service to maximise the opportunity to discharge people at the point they are ready to leave hospital. You would be integral in providing an excellent service to our patients easing their transition from hospital and ensuring that hospital beds are available for people that need them.

Every day, our Adult Social Workers are making a real difference to vulnerable people in our community, helping and supporting them to live their lives in the way they want to.


Salaries are offered at points within these bands depending on experience:

  • Social worker: £32,020 - £34,723
  • Advanced Social worker: £36,298 – £39,493

What will you be doing?

You will be working with people who require social care services, their families and carers to ensure independence in maintained as much as possible and outcomes are clear, monitored and reviewed.

You will:

  • deliver genuine people-centred, strengths-based support, with a focus on what those we work with tell us will make the biggest difference to them. using your creativity to deliver the best outcomes for local people.
  • implement, review and monitor actions and outcomes outlines in line with individual care plan.
  • make changes and amendments as the needs of the individual change and evolve
  • work in line with best practice legislation, policy and guidance to promote the wellbeing and dignity of service users
  • make recommendations for the implementation of reablement equipment

As an Advanced Social Worker you will be required to:

  • support students on placement as an experience Practice Educator
  • be fully up to date with the core training expectations as an Advanced Social Worker
  • use examples of research and evidence-based resources to enhance practice and share learning with team (e.g. use of Research in Practice / other online resources).

What we need from you

  • It is essential that you have must have a degree in Social Work with relevant registration to practice or be in the final year of your Social Work studies.
  • You should also understand asset and strength-based approaches to adult social care.
  • You will be a problem solver, confident about making important decisions, underpinned by a genuine passion to work collaboratively with people to meet their goals and aspirations.
  • You’ll welcome challenge and embrace change.

Advanced Social Worker:

  • We will require you to have a minimum of 2 years post qualification experience.
  • You will have evidence of meeting the essential (and desirable where possible) criteria for the role of Advanced Social Worker or in South Gloucestershire council.
  • A clear commitment to Continuous Professional Development (CPD)

You will be expected to travel throughout the authority and surrounding area so must have a full valid driving licence with regular access to a vehicle or have alternative appropriate means of travel.

Please note that due to the nature of this role it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained for this post
