1.1 Job Summary

Working as part of a multi-disciplinary team (MDT), the job holder will play a key role in supporting the PCN vision of creating a new wider Community Service, in collaboration with the full MDT, supporting the delivery of policy and procedures, meeting the needs of our patients.

This role plays a pivotal part within the community service, working with support workers and registered nurses throughout the PCN to deliver care for patients and the public, while seeking to improve the quality of care and operational efficiencies across the local healthcare system, demonstrating a passion to deliver excellent service within general practice.

As a Nurse Associate (NA), the role will work alongside doctors, physicians, GPs, registered nurses and other allied health care professionals to provide medical care as an integral part of the multidisciplinary team. NA’s will support the assessment of patient care needs and provide continuing care for the patients, working within the professional boundaries.

1.2 Key Working Relationships

Key relationships include but are not limited to:

· Patients, service users and their representatives

· GPs within the PCN practice

· Overall PCN and GP practice workforce

1.3 Key Responsibilities

Clinical and Patient Client Care

· Provide continuing care to patients across the practice/PCN population.

· Treatment room duties in line with competency e.g. Suture/clip removal, Basic & complex wound care, ECG. Phlebotomy, Urinalysis.

· Monitoring and support of the management of patients with long-term conditions / chronic diseases, using a holistic patient centred approach.

· Partake in Practice MDT, supporting the co-ordination and management of patients living in Care Home.

· Support patients to improve and maintain their mental and physical health and wellbeing.

· Assist in the provision of minor surgery, coil and implant fittings.

· Chaperone patients when required.

· Administering of Medication in accordance within regulatory guidelines for Nursing Associates.

· Empathise with, and support people to improve and maintain their mental and physical health and wellbeing.

· Assist with the implementation and monitoring of clinical care and monitor outcomes

· Contribute to ongoing assessment recognising when it is necessary to refer to others for reassessment.

· Be able to perform cardio-pulmonary resuscitation according to current UK guidelines.

Financial & Resources

· Foster and maintain strong links with all services across the PCN and neighbouring networks.

· To have full knowledge of the budget, ensuring best value for money, and identifying efficiencies as appropriate, ensuring senior managers are aware of the cost implications of any areas of non-compliance.

· To manage resources required for key duties under this Job Description, ensuring value for money at all times and maintain budget management responsibility allocated to the job holder in line with the scheme of delegation, develop, where required, cost-benefit analysis for spending and initiatives.

Research & Development

· Assist in the training and development of trainee Nurse Associates.

· Participate as required in any research studies taken on by the practice.

· Contributes toward the development and embedding of the BHNC and PCN visions, aims and business objectives.

· Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities.

· Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.

· Explore the potential for collaborative working and take opportunities to initiate and sustain key working relationships with stakeholders as needed for the collective benefit of patients.

· Work with your line manager to access regular ‘clinical supervision’, to enable you to deal effectively with the difficult issues that patients present.

Policy & Service Development

· Assist with the co-ordination of Infection Control policy and audit.

· Proactively provide feedback to you manager when patient care and service provision is impacted by changes to the PCN’s policies, either negative or positive, to support the continual development of the PCN.

· Feedback to your manager (or appropriate policy/service Lead) if you see a need or area of development within policies or service provision while carrying out your role.

· When required, support the evaluation of service provision.

· Work within policies regarding family violence, vulnerable children and adults, substance abuse and addictive behaviour, and makes referrals as appropriate for safeguarding.

o Detailed knowledge and understanding of appropriate procedures and treatment plans,

o Completion of Nursing Associate foundation degree qualification,

o Registered Nursing Associate with the NMC.

o Have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when what the person needs is beyond the scope of the link worker role,

Additional Requirements

o Ability to travel across the PCN area on a regular basis, including visiting GP practices, patients in their own homes,

If you are a manager or have line management responsibilities for staff, a department or area of work, you are responsible for the risk management issues in that area. In conjunction with risk management you will ensure that there is an annual risk management audit in your area, risks are identified on the local risk register and that where necessary, an action plan eradicating risks is drawn up and implemented.

Protection of Children and Vulnerable Adults

All employees have a duty for safeguarding and promoting the welfare of children and vulnerable adults. Staff must be aware of the organisational procedure for raising concerns about the welfare of anyone with whom they have contact. Clinical Governance

The CGG promotes an open, learning culture ensuring appropriate governance systems and processes are in place to support and develop this culture. The post holder is responsible for ensuring that they are aware and compliant with the CCG’s policies and procedures that govern their work; and if something goes wrong, they have an obligation to report it so lessons can be learned from mistakes, incidents and complaints.

If any member of staff has concerns on any clinical governance matters, they should raise them with their line manager, professional adviser, or a senior member of management.

Infection Prevention and Control

Employees are expected to work to minimise any risk to clients, the public and other staff from Healthcare. Associated Infection including MRSA and C difficle by ensuring they are compliant with the Health Act 2006 – Code of Practice for the Prevention and Control of Healthcare Associated Infections (They Hygiene Code); and by ensuring they are familiar with the practice Infection Control Polices within the practice.
