Job Description

We are looking for a Band 6 Speech and Language Therapist (SLT) for an inpatient rehabilitation post at St Pancras Rehabilitation Unit (SPRU) in Camden. This is a permanent post for 30 hours a week (Some flexibility can be negotiated regarding working hours). This post is a static post within St. Pancras Rehabilitation Unit and would suit a band 6 therapist wanting to consolidate established skills in the area of inpatient rehabilitation and neurology. Previous experience of working with patients who have a variety of neurological conditions is essential.

MAIN DUTIES AND RESPONSIBILITIES

Clinical
1. To thoroughly assess presenting patients through subjective questioning, objective formal and informal assessment and analysis of information gathered
2. To prioritise factors involved in a patient’s presenting condition through clinical reasoning based on knowledge of evidence-based practice, current professional opinion, the patient’s prognosis and other physical psychological and social issues.
3. To ensure high standards of clinical practice using evidence-based practice and national/local clinical guidelines
4. To advise and teach the relevant carers/health workers to participate in on-going Speech & Language Therapy rehabilitation care or disability management programmes for clients, as appropriate
5. To set goals for treatment with clients and/or their carer and ensure that care is client-centred and agreed by the client and/or their carer
6. To devise and manage a treatment plan individual to a client’s presenting condition through, for example the use of functional rehabilitation, compensatory approaches, provision of alternative and augmentative communication (AAC) and feeding equipment, education and health promotion
7. Act as an advocate on behalf of clients and carers by adopting a client-centred approach to support meaningful participation in life roles within an inpatient setting and the local communities
8. To ensure that the client and/or their carer have an understanding of treatment protocols and gives informed consent to examination and treatment
9. To ensure that the patient understands the clinical reasoning process and the management plan
10. To modify the patient’s therapy treatment/management plan as appropriate following ongoing re-assessment and evaluation of progress
11. To manage a caseload of clients with complex needs with supervision and guidance as necessary from senior staff. To successfully monitor, evaluate and review Speech & Language Therapy treatment/interventions within a multi-disciplinary framework and to modify as necessary taking a key worker role as indicated
12. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care
13. To have the ability, knowledge and skills required for moving and handling people in inpatient and community environments (as required for swallow assessments)
14. To ensure that all documentation is accurate, contemporaneous and meets the documentation policy of the Trust.
15. To prescribe, delegate and monitor the work of rehabilitation assistants as required
16. To manage clinical risk in own caseload and ensure that rehabilitation assistants and students comply g. regarding dysphagia and manual handling.
17. To actively be involved, as required in the development of new specialist clinical services within the team, with support and guidance from Senior Therapy staff
18. To contribute to the efficient, day-to-day, running of the team
19. To maintain case notes and write reports outlining recommendations for discharge, independently or jointly with other professions. These are communicated to external agencies g. GPs, Social Services and Community Therapists

Professional
1. To keep up to date with Speech & Language Therapy professional registration and clinicalpractice through CPD activities and be able to demonstrate a commitment to CPD as required of the HCPC
2. To develop a specialist knowledge base in rehabilitation and disability management, assessment, clinical reasoning and management skills
3. To evaluate your practice through audit and outcome measures
4. To access library services, medical databases and the Intranet to further develop clinical practice and service initiatives
5. To develop links and communicate with other members of the multidisciplinary team
6. To be professionally and legally accountable for all work undertaken
7.

Management
1. To ensure that accurate and evaluative records of patient care are maintained in accordance with the Trust/ Service/Speech & Language Therapy service policies
2. To manage a clinical case load supported by senior staff
3. To participate in the Trust’s appraisal and supervision schemes and contribute to the appraisal of rehabilitation assistants and Speech & Language Therapists with support from senior staff
4. To ensure that all equipment defects, accidents, incidents and complaints are reported to the line manager immediately and that appropriate action is taken following Trust and Service policies
5. To supervise junior members of staff, rehabilitation assistants and volunteers as required. To delegate administrative and clinical tasks appropriate to their competencies
6. To comply with the Trust’s policies and procedures, and the Health and Safety at Work Act (1974)
7. To attend and actively participate in multidisciplinary team meetings and case conference
8. To keywork patients and to manage handovers e.g. in periods of annual leave. To efficiently manage own time e.g. in respect to caseload, in-service training, supervision, project work and meetings
9. To input and retrieve computerised information
10. To participate in the collation of accurate statistics and reports required for monitoring and evaluating the service
11. To be involved with the implementation of the team’s clinical governance plan
