Hours: 30 – 37.5 hours
Salary: £39,000 - £43,000 depending on experience.
Job Summary
A vacancy has arisen for a Lead Nurse role in the surgery.
We are seeking a hard-working individual with exceptional leadership qualities, organisational skills and general practice knowledge to join our team. Time served as a Lead Practice Nurse or equivalent managerial experience is preferred.
The successful candidate should have a good understanding of the GMS contract and Practice Governance. Must be enthusiastic, highly reliable and motivated, and a dedicated team member who is able to work constructively with all members of the practice team.
Essential skills: travel and baby vaccinations, cervical screening, health promotion advice, and the management of long-term chronic conditions. Work will involve all general treatment room duties, housekeeping, administration duties, and supporting the clinical team and GP’s, as well as overseeing the team and being an active lead role within the practice.
Desired Skills:
Diploma in diabetes qualification or equivalent and ARTP Spirometry trained including interpretation of results.
The surgery consists of 16,000 patients, between two GP practices on a single site, in a centrally based location with a diverse population. The GPs are complimented by a large team which includes five Practice Nurses, two Health Care Support workers, and exceptional management and administrative support.

JOB DESCRIPTION – LEAD PRACTICE NURSE
Working in accordance with the NMC Code of Conduct, the post-holder willlead and develop the practice nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.
The post-holder will develop (and ensure that the nursing teamwork with reference to) practice-agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
A nurse with prescribing qualifications will use them appropriately.

Job responsibilities
Management responsibilities:
- Allocate, co-ordinate, monitor and assess the work of the nursing team and individuals.
- To manage the nursing service in participation with the practice manager ensuring relevant information is disseminated to and from the nursing staff.
- Supervise and assess staff performance and development and ensure compliance with mandatory training needs and revalidation for all nursing staff, in participation with the practice manager carry out yearly appraisals of all the nursing staff.
- Identify the workforce that is required to provide services to meet the needs of the patient population and negotiate with the practice management team for this workforce. Ensure the practice manager informed of any matters arising or problems / potential problems.
- Enable the nursing team to work effectively and provide a high-quality service to patients and to work effectively with other agencies.
- Enable the practice nursing team to apply and improve clinical governance and risk management systems.
- In conjunction with the Primary Care Team, assist and monitor the development and implementation of policies, standards and guidelines across the practice nursing team.
- Participate in recruitment, induction and training of new staff and participate in the teaching of medical students and trainee GP’s.
- Develop a working environment that promotes healthy, safe and effective work outcomes.
- Alongside appropriate delegates, general nurse administration and organisation of treatment room rotas.
Clinical Responsibilities / Diagnosing and treating patients presenting with minor illness:
- Triage and treat patients wishing to see a health care professional making any necessary referrals to other members of the primary health care team.
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
- Assess, plan, implement and evaluate individual treatment plans for patients with known long-term conditions.
- Proactively identify and manage treatment plans for patients at risk of developing long-term conditions.
- Key role in the planning, preparation, and delivery of seasonal vaccination clinics, including the health promotion, team training and sharing target and uptake data.
Providing assessment, screening treatment services and health education
advice:

- Assisting with minor procedures in the treatment room
- Organise and coordinate the provision of nursing services for the practice to include the delegation of duties to other members of the practice nurse team as appropriate
- Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary
Pathological specimens and investigatory procedures
- Undertake the collection of pathological specimens including phlebotomy, swabs etc. Perform investigatory procedures requested by the GPs.
Administration and professional responsibilities
- Participate in the administrative and professional responsibilities of the practice team.
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patient’s notes.
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended.
- Ensure appropriate items of service claims are made accurately, reporting any problems to the Practice Manager or appropriate deputy.
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
- Attend and participate in practice meetings as required.
- Restocking and maintenance of clinical areas and consulting rooms.
Supplies and equipment
- Manage and oversee the maintenance of equipment and stock relating to patient care.
Training and personal development
- Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals’ responsibility to remain up to date with recent developments.
- Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate.
- Develop and maintain an in house nurse-training programme reporting to the practice manager.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
- If it is necessary to expand the role to include additional responsibilities, full training will be given.
- Develop and maintain a Personal Learning Plan.
- Maintain NMC and Revalidation as required by the NMC.
Liaison
- As well as the nursing team there is a need to work closely with reception and administrative staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team.
- There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care.
Meetings
- It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
- In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential
- Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data
