1. Provide operational leadership and oversight of the field team to provide direct Member facing preventive care services to ensure services meet Member experience, cost effective, and quality assurance requirements are delivered to Members. This includes maintaining accurate knowledge and competence around various screening and assessment tools, health plan resources & benefits, and knowledge of systems/process to engage Members with their PCP or referral path forward (e.g., closed loop referral).
2. Leads appropriate trainings and/or practicum to ensure the team is able to roll out new activities with competency, efficiency, and consistency of work. (i.e. new screenings tools, member informing health navigation topics, etc.)
3. Monitors the workflow and ensures the team functions as part of an integrated care team to support overall Basic Population Health Initiatives.
4. Leads and supports the team to outreach and engage Members with their PCP, for appropriate utilization of preventive health care services with their PCP.
5. Ensures the team’s timely response to referrals for programs/services. In additional, monitors the appropriateness of the team referring forward for other health plan services or resources.
6. Support the team to ensure the delivery of services with cultural competency and consideration for disparate populations to close equity gaps.
7. Support Team Member Success and Engagement by setting goals and measures for successful program implementation; provides timely and relevant feedback to Team Member for continuous improvement; and removes barriers to support successful adherence and completion of strategic regional workplans.
8. Keep the Team informed of any programmatic and/or organizational changes across IEHP departments to ensure accurate information is being provided to all Members and Partners.
9. Demonstrate proficiency in utilizing monitoring systems (such as Cisco, MHK, MediTrac, SuperSearch and Web Portal). Conduct weekly quality assurance activities including but not limited to Verint Monitoring, Workforce Management Reporting. Communicates regular feedback to Team Member based on audits in Cisco or Verint System.
10. Model the importance of Member experience and engages the team to support process improvement activities, as well as support the delivery of activities and services that reflect IEHP’s Mission, Vision, and Values.
11. Partner with key stakeholders (internal and external) to establish and maintain workflow processes ensuring the delivery of high-quality services at specific project sites. Provide hands-on, on-site support to the team with events and activities, as needed.
