Job Description
We are looking for a highly motivated and dedicated Senior Support worker to join our team to provide support to nursing staff and other members of the MDT in the assessment, planning, implementing and evaluation of effective, evidenced based care, ensuring the care is holistic and that standards of care are enhanced.
You will perform dual role by working closely with Slough area care coordinator and cover duty to complete wellbeing assessment and other related back up support as may require.
The successful applicant will be work under the guidance of a qualified professional to provide quality care and support which enables clients to be supported with a recovery focus plan throughout their perinatal journey, whilst promoting independence and protecting choice, dignity, privacy and safety.
This post has been created to enhance the Perinatal service operations and care delivery to our birthing mothers, their baby and families.

Interviews to be held 25th May 2023
1. To contribute to the clinical management of designated service users, in assessing, planning, implementing, and evaluating care. Supporting and working collaboratively with the clients care coordinator in reviewing and updating risk assessments and risk management and care-plans with clients and their carers within their level of competency under the supervision of a registered professional.
2. Carry out assigned tasks and report the outcome and effectiveness of these activities to a qualified professional e.g., supporting interventions as outlined by MDT.
3. To understand the range of interventions available, the evidence base and potential outcomes for clients and apply appropriately.
4. To record all contacts with clients in their electronic records in line with RIO record keeping policy.
5. To develop and maintain effective professional relationships and communication channels with all members of the MDT, other health care professionals and agencies in order to ensure continuity of care and enhance recovery.
