Due to the promotion of the current post holder, we have an exciting Band 7 pharmacist role to offer.
The post holder working closely with the Home Based Treatment Team (HBTT) technician will be responsible for the provision and continued development of a clinical pharmacy service for the HBTTs, Manchester citywide.
The role will include providing pharmaceutical advice to medical staff, nursing staff and other members of the multidisciplinary team (MDT) and to patients and their carers.
Working within the pharmacy team we will offer you support to develop your expertise and skills through formal training courses, supervision, and regular appraisals. Our team has a wealth of clinical knowledge in a wide range of clinical specialities with a strong team of senior pharmacists. We have excellent links with local universities and as well as a variety of clinical national networks.
As a member of our team, you will be responsible for ensuring that drug therapy is evidence based, safe, cost effective and appropriate to the individual patient. Your role will also include ensuring that medicines management practices within the MDT and services comply with legal, national and local guidelines.

- To deliver specialist clinical pharmacy services to HBTTs
- To provide expert pharmaceutical advice to consultants, medical staff within the trust and in primary care plus other members of the multidisciplinary team (MDT).
- To provide expert pharmaceutical advice to patients and carers
- To ensure that drug therapy is evidence-based, safe, cost-effective and appropriate to the individual patient.
- To ensure that medicines management practices within MDT and services comply with legal, national and local guidelines
- To train and supervise pre-registration pharmacy graduates, pharmacy undergraduates and technical staff.
- To attend MDTs for each team on a regular basis.
- To prioritise patients with complex medication needs for medication
- histories, medication reviews and clinical input.
- To provide pharmaceutical care to the HBTT patients citywide providing a clinical pharmacy and prescription monitoring service ensuring safe and appropriate use of medicines in order to maximise benefit and minimise risk to patients.
- To act as an expert source of pharmaceutical advice for health and social care staff, patients and carers. To provide a clinical pharmaceutical opinion when medical information is conflicting.
- To recommend, substantiate and communicate pharmacological treatment options to consultant medical staff, GPs and junior medical staff.
- To recommend individualised, complex care-plans encompassing all relevant mental, physical and social care needs.
