Job overview

Seeking dynamic, highly motivated and pro-active nurses to join our enthusiastic, high performing and rapidly developing team of Specialist Respiratory Nurses. We have a unique opportunity to develop an integrated pathway for patients with airways disease, bringing together the resources and expertise of respiratory teams at RCHT, CFT and Virtual wards. This will enable the development of an early supported discharge pathway, enabling appropriate patients to benefit from supported recovery in their own homes.

The current RCHT team have responsibility for reviewing patients admitted with an exacerbation of COPD or Asthma, providing education and support for the patient, giving management recommendations to the ward-based MDT, and ensuring referral to community services/ GP on discharge. Our work will expand to assess suitability for early supported discharge with support from the virtual ward. We will provide an outreach service to support patients in their own home supported by the virtual wards.

You will have prior experience of working within a respiratory setting and be able to demonstrate a high level of knowledge and expertise of respiratory patient care. These attributes will enable you to work as an independent practitioner, whilst at the same time working closely with the multi-disciplinary team

You will have a flexible approach to work, participating in service development and evaluation as we move forward, with a solution focussed can do attitude.

Main duties of the job

The post holder will work within the Specialist Asthma, COPD & Tobacco Dependency team to provide a high standard of specialist care to patients who attend or are admitted to hospital with an exacerbation of airways disease.

  • to provide specialist respiratory nursing assessment and support to patients who attend ED or are admitted hospital.

-facilitate and participate in providing onward care in an appropriate setting ie - in hospital or home with support from virtual ward/ community services.

  • work in partnership with patients and carers to offer support and advice, promote informed choice and self-management.
  • work autonomously, providing effective, evidence based care in line with competency and organisational policies, procedures and guidelines.
  • work under the indirect supervision of the band 7 lead respiratory nurse specialist
  • oversee the caseload and delegate to other team members in the absence of the lead respiratory nurse specialist
  • work in an integrated way with all other agencies and other members of the multi-disciplinary team, across organisations
  • work in a flexible manner seeing patients in a variety of locations including hospital and patients’ homes.

The development of Virtual Wards for Respiratory Patients will support the delivery of integrated respiratory pathways which will improve the detection, diagnosis, management and rehabilitation of respiratory patients.

Education / Qualifications

Essential criteria
  • Registered Nurse (adult)
  • Qualification in Respiratory Care
  • Mentorship module or willingness to undertake
  • Hold a post-registration diploma or be working towards a degree
  • Experience of working in a respiratory setting

Skills and Aptitude

Essential criteria
  • Excellent communication and interpersonal skills
  • Working knowledge of spirometry, pulse oximetry and inhalers
  • IT skills to include use of work, excel spreadsheets, email, internet access
Desirable criteria
  • Arterial blood gas analysis
  • Teaching and presentation skills
  • Advanced computer skills
