An exciting opportunity has arisen for an enthusiastic Social Worker to join the new and innovative Eastern Home First Hub Team within the Eastern Locality to embed Discharge to Assess (D2A) and the NHSE Hospital Discharge Service Policy & Operating Model. Norfolk County Council are on an ambitious journey to embed Discharge to Assess as part of our Ageing Well Accelerator programme. We have already made great strides over the last few years and now need to take forward further rapid redesign in line with national policy and quality improvements to ensure adults being discharged from the James Paget University Hospital and community short term placements, are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged. The Homefirst Team are working towards an integrated care system, alongside Eastern Locality Primary Care Hubs and Occupational Therapists. This social work role will provide support with the Acute Hospital with transfers of care within the Virtual Homefirst Hub with our partners, providing Care Act Assessments, Mental Capacity Assessments/Best Interest Decisions; Carers Assessments and undertaking Safeguarding Enquiries . With integrated working, you’ll enjoy variety in your day and the opportunity to build strong working relationships with professionals across the county council, NHS and other partners. Who are we?
Our teams work with older people and adults with a physical disability. Our teams of social workers, assistant practitioners, occupational therapists and reablement staff from Norfolk First Response work across all five localities. In the east we work closely with colleagues from East Coast Community Health Care. Our senior management teams are made up of operational directors and heads of integrated care who either work for Norfolk County Council or Norfolk Community Health and Care (NCHC). It’s a really integrated environment.
We work as part of the Integrated Care System and are also aligned to Primary Care Networks which means the integrated team structures benefits hugely from great collaboration with primary care colleagues. Our teams are strong voices for social care and social work within our integrated model and ethos of prevent, reduce and delay. The teams promote a supportive and learning culture with reflective practice and other learning opportunities such as practice teaching and social work apprenticeships encouraged.
We also have an Integrated Home First Hub of integrated health and social care staff which facilitates discharges from hospital. Our dedicated team facilitates discharges from Norfolk acute, mental health and community hospitals. We are also part of the Norfolk Emergency and Admission Avoidance Team (NEAT) which is an integrated team with health colleagues to prevent admissions to hospital.

Great Yarmouth & Waveney locality
Parts of both Norfolk and Suffolk combine to make up this locality. Large swathes of the Broads National Park run from north to south, with windmills dotting the skyline and huge beaches forming the coastline, including the seaside towns of Hemsby, Caister-on-Sea and Southwold. Inland, Beccles and Bungay are beautiful historic market towns.
The largest Norfolk town in this locality is the popular seaside resort, market town and maritime port of Great Yarmouth, 21 miles east of Norwich and easily accessible by road or rail. Lowestoft in Suffolk is the most easterly town in England, 28 miles from Norwich. It is also a seaside resort, market town and maritime port with direct rail access to Norwich and also to London via Ipswich.
