An exciting opportunity has arisen for a Team Manager to join the Northern Locality Team in a permanent post. In this role you would be leading and managing a committed and motivated team of social care staff who work with older people and adults with a physical disability. You would be responsible for leading and supporting the team to deliver outstanding performance. North Place is now part of the Connecting Communities Environment for Social Care strategy, embedding the new ways of working into our daily practice and ensuring that strengths-based approaches are at the forefront of our work.

You will be working closely with community health managers and primary health care colleagues and will need to be a qualified and registered social worker to apply for the role. This is an integrated team but there is further work to be done to generate creative and sustainable opportunities to support the wider health and social care network. You will be responsible for managing a budget and ensuring the delivery of key performance measures, with the drive and motivation to support the team to work in ways that focus on an individual’s strengths and that promote independence. Supporting people to remain at home is the aim and team managers need to be resourceful and dynamic in their thinking, to support practice across the locality that leads to positive outcomes for the people of North Norfolk. You would be joining a supportive and forward-thinking locality management team and would have the opportunity to lead in specific portfolio areas and contribute to the development of local projects. In return you will receive regular supervision and training as well as support from your Team Manager peers and Head of Integrated Care, with opportunities for reflective practice and continuing professional development. We also support team members to consider post-graduate qualifications such as the Approved Mental Health Professional training, practice education award, or Best Interests Assessor award.

Who are we?

Integrated Community Teams Care incorporating Older People and People with Physical Disabilities

Our teams work with older people and adults with a physical disability. Our teams of social workers, assistant practitioners, Occupational Therapists and reablement staff from Norfolk First Response work across all five localities. In the east we work closely with colleagues from East Coast Community Health Care. Our senior management teams are made up of Operational Directors and Heads of Integrated Care who either work for Norfolk County Council or Norfolk Community Health & Care (NCHC). It’s a really integrated environment.

We work as part of the Integrated Care System and are also aligned to Primary Care Networks which means the integrated team structures benefits hugely from great collaboration with primary care colleagues. Our teams are strong voices for social care and social work within our integrated model and ethos of prevent, reduce and delay. The teams promote a supportive and learning culture with reflective practice and other learning opportunities such as practice teaching and social work apprenticeships encouraged.

We also have an Integrated Home First Hub of integrated health and social care staff which facilitates discharges from hospital. Our dedicated team facilitates discharges from Norfolk acute, mental health and community hospitals. We are also part of the Norfolk Emergency and Admission Avoidance Team (NEAT) which is an integrated team with health colleagues to prevent admissions to hospital.
