An exciting opportunity awaits the right candidate as NHS North East and North Cumbria Integrated Care Board (ICB) seeks to recruit an experienced and self-motivated individual to join the Children and Young People Commissioning Team in Tees Valley as a Special Educational Needs and Disabilities (SEND) Health Facilitator. Working in a busy, high-volume but friendly and supportive environment, this role will suit an organised person with an eye for detail and plenty of team spirit .By joining our team in Tees Valley Integrated Care Partnership (ICP) you will be supporting the continued development of high-quality health provision to improve health outcomes and reduce inequalities for the Children and Young People of Hartlepool, Stockton-on-Tees, Darlington, Middlesbrough and Redcar and Cleveland. The Special Educational Needs and Disabilities (SEND) Health Facilitator is a key role in supporting the Designated Clinical Officers to ensure delivery of our objectives outlined in the Children's and Families Act (2014) and the SEND Code of Practice (2015) and will be responsible for supporting children, young people and their families to navigate and receive the health services they need at the earliest possible opportunity.
