Job overview

An exciting opportunity has arisen for several B6 practitioners to work within the newly formed and innovative Hospital and care at home service on the Oldham site. The post holder will work within the Hospital and care at home team alongside the multi-disciplinary team to provide and maintain a high level of service.

The current pathways are related to respiratory, frailty, and acute medicine infection pathways, with cardiology pathways in development therefore you should have experience within one of these specialties.

We are looking for enthusiastic and motivated individuals to join the service with a range of skills and clinical expertise and will consider applicants from any of the following backgrounds, Clinical Pharmacy, Registered nurses, and Allied health professionals. You need to be flexible, hardworking, have excellent communication skills and appreciate the need for professional development within your practice to ensure we establish a responsive and effective team.

Main duties of the job

You will be responsible for triaging, onboarding, monitoring, and escalating patients via the MDT on H&C@H pathways. You will need to be enthusiastic, motivated and an appropriately experienced practitioner. You will need to be a registered health care professional who can demonstrate your commitment, enthusiasm, and drive to develop your own clinical, managerial and team working skills within the service.

You must be able to communicate effectively at all levels, work autonomously and be able to demonstrate effective teamwork. You will provide high quality care in an appropriate and timely manner which is responsive to patients need whilst adhering to recognised guidelines.

Detailed job description and main responsibilities

The post holder will work within the Hospital and care at home(H&C@H) team alongside the multi-disciplinary team to provide and maintain a high level of service to service users accessing NHS services.

  • The post holder will take part in co-coordinating referrals, triage, initial assessments, onward referrals, and input into the MDT for patients accepted onto the H&C@H virtual ward
  • The post holder will be responsible for determining appropriate patients are onboarded onto the H&C@H virtual ward
  • The post holder will deliver, implement, monitor, and escalate patients on the H&C@H virtual ward.
  • They will ensure appropriate pathways of care and communication via liaison and referral to other agencies and specialist teams via the MDT as required.

The post Holder will ensure compliance with Professional policies, National and local guidelines and Trust policies, procedures, and protocols.

Key Roles and responsibilities:

  • Responsible for prioritising and planning own workload on a day-to-day basis with some direction.
  • To use advanced knowledge and skills in order to manage patients care as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience.
  • Negotiate, agree expectations, and set appropriate realistic and individual goals in partnership with the patient, carer and colleagues
  • Ensure that assessments, programmes of care, and evaluations related to patients being accepted and onboarded onto the H&C&H pathways comply with the Trust’s agreed clinical standards.
  • Ensure provision of holistic and rapid assessment of patients referred in a timely manner to support early discharge from the acute hospital provision.
  • Provide home visits and deliver programmes of care according to clinical need within agreed protocols after escalation and discussion at MDT meeting
  • Facilitate safe and timely discharge from the service including generation of a discharge summary to GPs.
